What is Premature Ejaculation?

Premature Ejaculation is a male sexual problem which causes sperm to expel too quickly.  It is a common sexual disorder in men younger than 40 years, although the condition can affect men of all ages. The American Urological Association (AUA, 2000) defines Premature Ejaculation ‘As the ejaculation that occurs sooner than desired, either before or shortly after penetration, causing distress to either one or both partners’. Being unable to delay ejaculation can lead to distress, frustration and /or the avoidance of sexual intimacy.

The different types of Premature Ejaculation

It is understood that there are two different types of Premature Ejaculation. Primary Premature Ejaculation first becomes noticeable once a male becomes sexually active. Whereas Secondary Premature Ejaculation occurs later on in life and is believed to be caused by psychological and physical factors. This condition may occur with all partners and situations or only with a specific partner or certain situation.

The causes of Premature Ejaculation

There is no clear cause of Premature Ejaculation. It is often considered to be caused by a combination of both physical and physiological problems. Anxiety, stress or problems with your sexual partner can often be a cause of premature ejaculation. Premature Ejaculation is common if you’re young and in the early stages of your first relationship, or if it’s been a long time since you’ve last ejaculated. Young men can often struggle to delay ejaculation. However, with sexual experience and age, the majority of men learn to delay orgasm.

The physical causes of Premature Ejaculation include prostate disease, an under-active thyroid, recreational drugs and alcohol. 

Treating Premature Ejaculation  

Although premature ejaculation is considered one of the most common male sexual problems it is often an undiagnosed and an under-treated condition. Very few patients seek medical advice to treat premature ejaculation, as they find the condition embarrassing. However, it is estimated that 20%-30% of men suffer from premature ejaculation. 

The assessment involves clinical history and examination. Prostate operations are rarely required and depend on the clinical findings. There are wide-ranging treatments including behavioural, psychological therapies and medications with variable success.
